Working Requirements & Current Job Openings
Applications are accepted from Mondays thru Thursdays 10 A.M. to 2:00 P.M.
All Documents MUST be an original no copies or faxes are accepted.
- Copy of Professional Licenses / Certificates 2
- Resume
- Current CPR Card
- Drivers License or Picture ID
- Social Security Card
- Employment Authorization (e.g. Alien Card, Voters Registrations Card, Passport or Work Permit)
- Current HIV CEU
- Domestic Violence
- Alzheimer CEU 1
- For all C.N.A and HHA an additional 12 Hours of In-Services is needed each year e.g. OSHA, Residents Rights etc. (NO Assisting with Medication) 1
- Current Physical within the last six (6) months signed by a doctor which states you are free of any kind of communicable diseases including Tuberculosis.
- Copy of car insurance
- Auto Policy
- Professional Liability Coverage for C.N.A/LPN/RN can also be completed online at www.cmfgroup.com
- Level 2 background check.
